The Transitioning Youth (TY) Program supports young adults with developmental disabilities as they move from school-based services into adult life. Designed for individuals between the ages of 18 and 21, this program focuses on building the skills, confidence, and opportunities needed for greater independence, employment readiness, and meaningful participation in the community.
Compassionate Home Health Residential Services, Inc. supports transitioning youth (21 years old) with developmental disabilities in Maryland, focusing on employment, independence, and community integration. Our team works closely with participants to help them prepare for adult life through practical skill-building, community involvement, and goal-oriented support.

Governor’s Transitioning Youth Initiative
The Governor’s Transitioning Youth Initiative was created to ensure that eligible students with developmental disabilities receive the services they need as they exit the school system. Through collaboration between the Developmental Disabilities Administration (DDA) and the Division of Rehabilitation Services (DORS), funding is available for supported employment and other day services for qualifying graduating students.
Without this initiative, many students leaving school could face long waiting lists before receiving adult services. The program ensures that funding is set aside within the DDA budget to support eligible students transitioning out of the school system, regardless of the level of their disability or the urgency of their service needs.
Eligibility for Transitioning Youth Services
To qualify for services under the Transitioning Youth Initiative, individuals must be determined fully eligible for developmental disability services. According to eligibility criteria, the disability must:
- Result from a physical or mental impairment, other than a diagnosis solely related to mental illness, or a combination of impairments.
- Be identified before the individual reaches the age of 22.
- Be expected to continue indefinitely.
- Limit the individual’s ability to live independently without ongoing assistance or external support.
- Require coordinated services such as specialized care, treatment, or other individualized supports.
Students may qualify for services under the Transitioning Youth category from their 21st birthday until their 22nd birthday. If a student graduates after turning 21, eligibility may extend for one year following graduation.
